Matthews Free Medical Clinic was awarded a one-year $80,000 grant in 2017 to 
expand their Screening and Care Program by hiring an additional nurse practitioner and medical assistant. Our grant enabled 481 individuals to be served. Program outcomes included 100% of new diabetic patients receiving an A1C test, 100% of new patients being offered appropriate specialist and ancillary services, and 100% of eligible new female patients being offered gynecological screening and services. Matthews Free Medical Clinic connected with five new community partners during our grant period and was able to put a streamlined new patient referral process in place.
